Therapeutic Intake Process at Relate

When you contact Relate Counseling Center to make your first appointment for outpatient therapy, one of our intake coordinators will help you determine which of our clinicians would be the best fit. You will have the chance to ask questions, make requests, and share your reason for seeking services. This information, along with your scheduling limitations and preferences, will be considered as the most appropriate clinician is selected for you. 

When possible, the intake coordinator will help you schedule your first appointment (called the intake appointment) upon your initial request for services. If further information is required from clinicians before the intake appointment can be scheduled, we will let you know the expected timeline for hearing back and finalizing your appointment. 

After your initial appointment date and time has been determined, we will send you electronic intake paperwork via email that you can fill out online prior to coming in. A member of our billing team will reach out to you to provide you with an estimation of costs based on your insurance plan. You will also receive an invite from our client portal from “portal@procentive” where you can pay your bill, message your provider, and have access to various other documents. 

When you arrive at the office for your intake appointment, our staff at the front desk will confirm that we have all the necessary insurance information on file and check you in for your appointment. 

Your intake appointment serves as the opportunity for your therapist to gather as much information as possible about your presenting concerns and to get a feel for various areas of your life including, for example, your personal and family history. This information will be used to formulate treatment options and recommendations, which will develop into your “treatment plan.” (For clients aged 12 and younger, many clinicians prefer to gather this information by meeting with parents or guardians without the client present prior to meeting with the child.) 

Once you have established care with your clinician, you can discuss future scheduling with them directly. 

 

No Shows/Late Cancellations: If you cannot keep the appointment time reserved for you, please notify our office at least 24 hours in advance. If we are not notified in advance, you may be charged a no-show fee for the missed appointment.
