When you call Relate to make an appointment for outpatient therapy, our intake coordinator will assist in determining which of our clinicians would be the best fit for you. During this first call, you will have the opportunity to ask questions, make requests, and share a bit about your reason for seeking services. This information, along with your scheduling limitations, will be taken into consideration as the most appropriate clinician is selected for you. When possible, the intake coordinator will help you schedule your first appointment (called the intake appointment) during that first call. If further information is required from clinicians before the intake appointment can be scheduled, we will let you know the expected timeline for hearing back and finalizing your appointment.

After you have determined the date and time of your intake appointment, we will send you intake paperwork electronically that you can fill out online prior to coming in.

When you arrive at the office for your intake appointment, our staff at the front desk will confirm that we have all necessary insurance information on file and will ask you to fill out a Symptom Checklist and Family History form. In order to ensure that you have time to complete this process, please plan to arrive 30 minutes prior to your scheduled appointment time.

Your intake appointment serves as the opportunity for your therapist to gather as much information as possible about your presenting issues and to get a feel for various areas of your life including, for example, your personal and family history. This information will be used to formulate treatment options and recommendations, which will develop into your “treatment plan.” (For clients age 12 and younger, many clinicians prefer to gather this information by meeting with parents or guardians without the client present prior to meeting with the child.)

Once you have begun working with your clinician, you will be able to discuss all future scheduling with them directly.

No Shows/Late Cancellations: If you are unable to keep the appointment time that has been reserved for you, please notify our office at least 24 hours in advance. If we are not notified in advance, you may be charged a no-show fee for the missed appointment.
